Financial Assistance

United General Hospital offers financial assistance to qualifying patients, in accordance with state law.  You may contact a financial counselor with any questions on this program.  Our financial assistance application is attached.  You may complete this document and mail it back (with the requested information) to the hospital for consideration under this program.  Your application will be reviewed and a written response mailed back to you.
